0010250: Companions screen lists a dead companion far to the east and up, regardless of longitude
While wandering around, I randomly checked the companions screen and noticed that one of my former companions, from whom I had gotten separated some time ago, was nearby. I traveled in that direction, periodically checking the companions screen, and reached the ocean with Thel still listed as being somewhere east and above. Moving a short distance north or south changed it to ENE/ESE throughout the trek.

I then enabled fastdwarf and began to swim east, no longer seeking an old companion but answers. At some point a few world-map tiles out from shore I noticed Thel's entry now listed him as DEAD, in purple. Taking this as a sign that I was getting close, I pressed on. However, when I checked again later, he was no longer listed as DEAD, and was still somewhere to the east.

I swam almost to the edge of the world with no change before giving up, concluding that it's most likely a bug with the UI rather than a dead companion beckoning to me from Valinor, especially since that companion wasn't an elf and Valinor is supposed to be in the west anyway.

One is from the shore, the other is from near the edge of the world.
1. Load either of the attached saves

2. Press [c]

3. Note the listed direction of Thel Ivakpibo

4. Repeat with the other attached save

5. Note the vast difference in location and the severe lack of change in Thel Ivakpibo's reported location
I can't completely rule out the possibility that companion-order has something to do with it, but nor do I have any particular reason to believe that it does.
