0001220: Combat log for creatures doesn't record the creature's death.
I'm not sure if this is limited to forgotten beasts or if it shows up on most creatures. When I go through the combat log of a creature that has died, it doesn't show any information on how it was slain at the end. It just shows who last attacked it and ends.

On a related note, it would be nice to have the names of the dwarves that are fighting in the combat log as opposed to just "axedwarf". It'd be easier to tell which dwarves are kicking ass and which ones aren't.
This has pretty much been the case in all the combat logs I've seen for dead creatures but I haven't looked at enough of them to be sure it happens all the time.

Maybe it has something to do with the fact that the announcements log and the reports get eaten way too fast? Which is what I'm seeing in the arena.

No. The death announcement doesn't show up in the log at all.

I second that, last entry is usually a finishing blow, like hit in a head with it going a high arc, but no death statement.

I've been playing in 34.05 for five in-game years. There have been plenty of deaths, but in my logs I only just got my first combat log death report, a shot from a magma crab:

The spinning basalt laced with Chrilbis's kobold blood strikes The Stray Puppy in the head, bruising the muscle, jamming the skull through the brain and tearing apart the brain!
The Stray Puppy has been knocked unconscious!
The Stray Puppy (Tame) has been shot and killed.

Shooting, bleeding, suffocation and infection seem to generate a message.

Death by loss of vital organ (or half the body) or just plain damage (for creatures that have no vital organs) do not.

Another issue with the logs is the "blah blah dwarf stands up" repeated several times, even minutes after the battle is over. Anyone else notice that?

I second this issue, no death conformations for me too.
I heard it is connected to "witnesses" (no message if no one can see the death), but in case of a goblin head being severed by dwarf (clear witness here!) - it is definitely a bug.
And in my experience, shooting, bleeding, suffocation and infection mentioned earlier are also missing pretty often. (Having 10-dwarf marks-squad, i still didn`t have at least ONE "shot and killed" message.
