0004298: Dead squads are not removed from squad list, instead leaving an annoying placeholder

Description

The military screen lists militia captains who are dead, and whose squad members are all dead. This is ridiculous. When all members of a squad have been killed, the squad should be removed from the squad list.
Right now, my military screen lists "captain of the guard", "militia commander", and 6 "militia captains". None of those dwarves exist. I never chose a captain of the guard, and my militia commander and all my militia captains got killed.
At one point though, I had a militia captain still alive, and it annoyed me greatly that I couldn't see where he was on the list of 5 "militia captains", most of whom were dead. I had to "feel" my way through the list by repeatedly trying to create a squad with each captain until I found the one who wasn't dead. (I couldn't see anything about militia captains on the Nobles screen, because my militia commander was dead.)
This is very annoying.
